Terms & Conditions

Introduction

 

Welcome to HH Rehab Services. These Terms and Conditions ("Terms") apply to your use of the HH Rehab Services website and, where applicable, the provision of occupational therapy and hand therapy services.

 

Please read these Terms carefully before making a booking or using our services. By making a booking, you confirm that you have read and understood these Terms and agree to be bound by them.

Nothing in these Terms affects your statutory rights as a consumer.

 

These Terms and Conditions apply to services provided by HH Rehab Services ("HH Rehab Services", "I", "me" or "my") to individual clients, companies, case managers, insurers, solicitors and other organisations ("the Client" or "the Referrer").

 

By booking an appointment or engaging HH Rehab Services, you agree to the following Terms and Conditions. If you do not agree, please do not use the website or book for any services.

 

Definitions

  • “Services” refers to the occupational therapy and hand therapy services provided by HH Rehab Services.

  • “Practitioner” refers to the occupational therapist providing the Services through HH Rehab Services.

  • “Client” refers to the person receiving or intending to receive the Services from HH Rehab Services.

  • “Website” refers to the HH Rehab Services website at hhrehab.co.uk.

  • “You” and “Your” refer to any person accessing the Website or booking, receiving or otherwise engaging with the Services. Where applicable, “You” and “Your” may refer to the Client or the person booking the Services on the Client’s behalf.

  • "We", "Us" and "Our" refers to HH Rehab Services, an independent occupational therapy service operated by Helen Hyde, HCPC registered occupational therapist (registered HCPC name Helen M Rogerson).

 

Use of the Website and Bookings

 

You must provide accurate and complete information when making a booking or contacting HH Rehab Services.

 

Where a booking is made on behalf of another person, the person making the booking confirms that they are authorised to provide the information required and agree to these Terms on behalf of the person receiving the Services where appropriate.

 

You must not use the Website for any unlawful purpose or in a way that could damage, disable, overburden or impair the Website or interfere with its security.

We reserve the right to decline a booking where we reasonably consider that we are unable to provide an appropriate or safe service, or where the requested service falls outside professional scope of practice.

 

By using this Website, you confirm that you are eighteen (18) years of age or older or that you are using the Website under the supervision of a parent or legal guardian

Intellectual Property

 

Unless otherwise stated, the content of the Website, including text, graphics, logos, photographs and other materials, belongs to HH Rehab Services or its content suppliers and is protected by applicable intellectual property laws.

 

You must not reproduce, distribute, modify or commercially exploit Website content without our prior written permission, except where permitted by law.

Limitation of Liability

 

HH Rehab Services shall not be held liable for any direct, indirect, incidental, or consequential damages arising from:

  • The use or inability to use the Website.

  • Errors, omissions, or inaccuracies in the Website Content.

  • Access to or reliance on third-party websites linked to our Website.

  • Technical issues, disruptions, or viruses affecting your equipment, software, or data during Website usage.

 

​Nothing in these Conditions limits or excludes our liability for death or personal injury resulting from our negligence or for fraudulent misrepresentation.

Governing Law and Jurisdiction

 

These Terms and Conditions, as well as your use of the Website, are governed by and construed in accordance with the laws of England and Wales. Any disputes arising under or related to these

Conditions will be subject to the exclusive jurisdiction of the courts in England and Wales.

 

Changes to these Terms

 

We may update these Terms from time to time to reflect changes to our Services, legal or regulatory requirements, or the way in which we operate our business.

The current version of these Terms will be published on our Website with the date on which they were last updated.

 

Changes will not retrospectively alter the rights or obligations relating to a booking or contract that has already been agreed, unless required by law.

Where a change materially affects an existing booking or agreement, we will provide reasonable notice where appropriate.

 

Privacy and Personal Information

 

Your personal information will be handled in accordance with our Privacy Notice, which explains how HH Rehab Services collects, uses, stores and protects personal information.

The Privacy Notice also explains the lawful bases relied upon for processing personal information and your rights under applicable data protection legislation.

Our Privacy Notice is available on the HH Rehab Services website.

 

1. Scope of Practice and Clinical Information

 

HH Rehab Services provides Occupational Therapy and rehabilitation services within professional scope of practice, training, qualifications and clinical experience, in accordance with the standards and requirements of the Health and Care Professions Council (HCPC).

An initial assessment will be undertaken to understand your needs, identify your occupational therapy goals and determine whether the service provided is appropriate for you. Further assessment and review may be undertaken throughout the course of intervention where clinically appropriate.

Occupational therapy intervention will only be provided where it falls within the practitioner’s professional scope of practice, knowledge, skills and experience, and where it is considered appropriate and safe to do so.

 

If, following assessment or during the course of intervention, it becomes apparent that your needs fall outside the scope of practice, competence or the services that can appropriately be provided, the practitioner may decline to commence or may discontinue occupational therapy intervention.

 

Where appropriate, you may be advised to seek assessment, treatment or support from another suitably qualified healthcare professional or service. The decision to initiate, continue, modify or cease intervention will be based on professional judgement, the individual’s needs and relevant professional standards.

 

Information Provided by the Client and/or referring organisation

 

Clients and/or referring organisation are expected to provide information that is accurate and complete to the best of their knowledge and to inform the practitioner of any relevant changes during the course of intervention.

 

This may include, where relevant:

  • medical and occupational history;

  • current or previous diagnoses;

  • medications and relevant changes to medication;

  • relevant investigations, assessments or reports;

  • current or planned treatment or intervention from other professionals;

  • relevant health, functional, psychological or social circumstances;

  • relevant risks, allergies or other factors that may affect the provision of occupational therapy; and

  • any significant changes in circumstances that may affect the client’s needs or the safety or appropriateness of intervention.

 

Clients should inform the practitioner if information previously provided is inaccurate, incomplete or has changed.

 

The practitioner will use the information provided, together with their own assessment and professional judgement, to determine the appropriateness and safety of occupational therapy intervention. Failure to provide relevant information may affect the practitioner’s ability to assess, advise or provide intervention safely and appropriately.

HH Rehab Services/the practitioner will only provide assessment and treatment where it is considered safe, clinically appropriate and within professional scope of practice.

 

2. Fees and Payment

 

The current fees for services provided by HH Rehab Services are:

  • 50-minute Occupational Therapy or rehabilitation appointment: £80

  • 75-minute Occupational Therapy or rehabilitation appointment: £120

  • Report writing: £120 per hour

  • Urgent report writing required within 48 hours: £180 per hour

 

The time required for report writing will be charged based on the actual time spent preparing the report.

Additional services, including case discussions, liaison with other professionals, telephone consultations, additional administrative work or other work requested outside the agreed service, may be charged separately at the applicable hourly rate.

 

All fees will be confirmed at the time of booking or prior to the commencement of services.

Individual Clients

 

For individual clients, the full appointment fee must be paid at least 48 hours before the scheduled appointment in order to secure and confirm the booking, unless an alternative payment arrangement has been agreed in writing in advance.

 

An appointment will not be considered confirmed or secured until payment has been received.

Where payment has not been received at least 48 hours before the scheduled appointment, HH Rehab Services reserves the right to cancel or release the appointment and make the appointment available to another client.

 

Payment for individual appointments should be made using the payment method provided by HH Rehab Services at the time of booking.

Clients booking less than 48 hours before their scheduled appointment will have payment agreed with HH Rehab Services prior to this booking.

 

Companies, Case Managers, Insurers and Other Organisations

 

The requirement for payment 48 hours in advance does not apply to companies, case managers, insurers, solicitors or other organisations engaging HH Rehab Services under agreed invoicing arrangements.

 

For these organisations, invoices are payable within 30 days of the date of the assessment, unless alternative payment terms have been agreed in writing in advance.

Booking

 

When you request an appointment and agree to the appointment details, including the applicable fees and cancellation policy, you are entering into an agreement with HH Rehab Services for the provision of the Services

 

3. Cancellation and Rearrangement by the Client

 

If an individual client needs to cancel or rearrange an appointment, at least 24 hours' notice must be provided.

Where an appointment is cancelled or rearranged with less than 24 hours' notice, the full appointment fee will be charged.

Where an appointment is missed without notice ("non-attendance"), the full appointment fee will be charged.

 

The cancellation fee will not normally be refundable or transferable to another appointment.

 

Exceptions may be considered at the discretion of HH Rehab Services in circumstances that are considered exceptional.

For appointments arranged through a company, case manager, insurer, solicitor or other organisation, these same cancellation/rearrangement terms and conditions apply.

Where an appointment is cancelled with sufficient notice, HH Rehab Services will aim to offer a suitable alternative appointment where possible.

 

Late Arrivals & Missed Appointments

 

Clients are expected to arrive on time for their scheduled appointment. A 30-minute grace period will be allowed from the scheduled appointment start time.

If you arrive more than 20 minutes late, HH Rehab Services reserves the right to treat the appointment as a no-show and decline to provide the service. Alternatively, at the discretion of HH Rehab Services, you may be asked to rebook your appointment for another available date.

 

Where an appointment is deemed a no-show or cannot be completed due to late arrival, the payment made for the appointment will be forfeited and will not be refunded or transferred to a future appointment.

 

If you are running late, please contact HH Rehab Services as soon as possible. While we will always do our best to accommodate delays, this cannot be guaranteed and appointments may need to be shortened, rescheduled, or cancelled where the delay affects the remainder of the day's schedule.

 

4. Cancellation or Rearrangement by HH Rehab Services

 

In exceptional circumstances, it may be necessary for HH Rehab Services to cancel or rearrange an appointment.

HH Rehab Services will provide as much notice as reasonably possible where there is a need to cancel an appointment and will aim to rebook the appointment at the earliest suitable opportunity.

Where an appointment has been paid for in advance and HH Rehab Services is required to cancel the appointment, the client will be offered the choice of rearranging the appointment or receiving a refund of the appointment fee.

HH Rehab Services will not be liable for any indirect costs, losses or expenses incurred by the client or referring organisation as a result of an appointment being cancelled or rearranged.

 

5. Home Visits

 

Where occupational therapy is provided in a client’s home, the fee for a home visit is £130 per hour. The applicable fee will be confirmed at the time of booking.

Travel and mileage costs are calculated separately on an individual basis, taking into consideration the location of the home visit and the travel required. Any applicable travel or mileage charges will be discussed and confirmed with the client prior to the appointment.

 

The client is responsible for providing reasonable access to the property and a safe and suitable environment in which the assessment or intervention can take place.

Before and during a home visit, the practitioner may consider whether the environment is suitable and safe for the proposed assessment or intervention. This may include consideration of access, hazards within or around the property, pets, smoking, the presence of other individuals, infection risks, moving and handling requirements and any other circumstances that may present a risk to the client, practitioner or others.

 

The client should inform the practitioner in advance of any known circumstances that may affect the safety or suitability of the home visit, including the presence of animals, significant environmental hazards or other relevant risks.

 

The practitioner may request reasonable measures to reduce identified risks before or during a visit. Where a safe working environment cannot be established, the practitioner may decline to undertake or may end the home visit and, where appropriate, consider alternative arrangements.

 

The practitioner reserves the right to leave a property immediately where they reasonably believe that their safety or the safety of another person is at risk, including in circumstances involving threatening, abusive or aggressive behaviour.

 

The same cancellation terms and conditions that apply to appointments also apply to home visits. This includes cancellation, rearrangement, failure to attend, or failure to provide reasonable access to the property at the agreed appointment time. Any applicable cancellation or non-attendance fee will be charged in accordance with the practice’s Cancellation Policy.

 

Where a home visit cannot safely or appropriately proceed, the practitioner may recommend an alternative appointment arrangement, such as a remote consultation, clinic-based appointment or referral to another appropriate service, where available and clinically appropriate.

 

6. Client Responsibilities

 

Clients are expected to:

  • Provide accurate and complete information relevant to their health and treatment.

  • Inform HH Rehab Services of any changes in their medical condition, medication or circumstances.

  • Provide relevant medical records and information where requested.

  • Follow agreed safety advice and rehabilitation recommendations.

  • Inform HH Rehab Services if they experience a significant deterioration or change in symptoms.

  • Attend appointments at the agreed time and location.

  • Provide at least 24 hours' notice if they need to cancel or rearrange an appointment.

 

Clients should not undertake exercises or rehabilitation activities that have been advised against by their medical or healthcare team.

 

Where behaviour presents a risk to safety or prevents the practitioner (HH Rehab Services) from providing occupational therapy safely and effectively, the appointment may be ended and/or discontinue the occupational therapy service.

 

Where reasonably practicable and clinically appropriate, the practitioner will explain the reason for discontinuation and, where appropriate, advise the client of alternative sources of support or care.

 

7. Limitations of Treatment

 

While HH Rehab Services will provide assessment and treatment with reasonable skill and care, rehabilitation outcomes cannot be guaranteed.

Recovery and progress can be affected by a number of factors, including the underlying medical condition, injury, surgery, individual healing, adherence to rehabilitation, general health and other circumstances outside the control of HH Rehab Services.

The provision of rehabilitation services does not guarantee a particular clinical outcome.

 

8. Reports and Written Documentation

 

Where a written report is requested, the report will be prepared based on the information available to HH Rehab Services at the time of writing.

The client and/or commissioning organisation is responsible for ensuring that all relevant information required for the report has been provided.

Reports are intended for the agreed purpose and recipient. They should not be altered or relied upon for a purpose other than that for which they were prepared without prior agreement from HH Rehab Services.

 

Report writing is charged at £120 per hour.

 

Where a report is requested and required within 48 hours, the urgent report-writing rate of £180 per hour will apply.

Additional charges may apply for amendments, substantial revisions or additional work requested outside the original agreed service.

 

9. Confidentiality and Information Sharing

 

HH Rehab Services will maintain the confidentiality of client information in accordance with applicable professional standards and data protection legislation.

Information will only be shared with relevant individuals or organisations where appropriate and lawful, including where consent has been provided or where there is a legal or professional obligation to do so.

 

Where a client is referred or funded by a case manager, insurer, solicitor or other organisation, relevant information regarding the assessment, treatment and progress may be shared with the commissioning organisation where this has been agreed and appropriate consent is in place.

10. Professional Boundaries

 

HH Rehab Services is committed to providing a professional, respectful and safe service.

Clients and professionals working with HH Rehab Services are expected to communicate respectfully. HH Rehab Services reserves the right to discontinue services where behaviour is threatening, abusive, discriminatory or otherwise inappropriate.

 

11. Complaints and Concerns

 

HH Rehab Services aims to provide a high-quality, professional service. If you have a concern or complaint regarding the service provided, please raise this with HH Rehab Services as soon as possible so that the matter can be considered and addressed appropriately.

 

12. Changes to Terms and Conditions

 

HH Rehab Services reserves the right to update these Terms and Conditions from time to time. The version applicable to the client or commissioning organisation will be the version in place at the time the service is agreed, unless otherwise communicated.

 

13. Acceptance of Terms

 

By booking an appointment or engaging HH Rehab Services, the client and/or commissioning organisation confirms that they have read and understood these Terms and Conditions and agree to comply with them.
