Specialist Occupational Therapy Support When You Need It

As a case manager, you coordinate a wide range of services to ensure your client receives the right support at the right time. Finding professionals who can provide high-quality, flexible and clinically informed occupational therapy input can make a real difference to the rehabilitation journey.

At HH Rehab Services, I provide independent occupational therapy services to support case managers, solicitors, insurers and individuals following injury, illness or changes in physical function.

With 19 years of NHS experience across a range of clinical settings, alongside experience in occupational therapy and hand therapy, I can provide tailored assessment, rehabilitation and advice based on each individual's needs and goals.

How Can I Support Case Managers?

I can work alongside case managers and the wider multidisciplinary team to provide additional specialist occupational therapy input where it is needed.

Depending on the client's needs, this may include:

  • Functional assessments to understand how an injury or condition is affecting everyday activities.

  • Occupational therapy assessments in the home or community environment.

  • Upper limb and hand therapy assessment and rehabilitation.

  • Assessment and treatment of difficulties affecting hand function, strength, dexterity and fine motor skills.

  • Functional rehabilitation to help clients regain independence and confidence in daily activities.

  • Advice and rehabilitation following orthopaedic injuries, neurological conditions and spinal conditions.

  • Support with identifying appropriate rehabilitation goals and developing practical strategies to work towards them.

  • Advice regarding equipment, adaptations and strategies to improve independence and safety.

  • Recommendations for ongoing rehabilitation and exercises that can be incorporated into the client's daily routine.

  • Progress reviews to assess how a client is responding to rehabilitation and whether their goals or treatment programme need to be adapted.

  • Clinical reports and written recommendations to clearly communicate assessment findings, progress and recommendations.

  • Liaison with case managers and other professionals involved in the client's care to ensure rehabilitation is coordinated effectively.

My approach is always person-centred and goal-focused, with recommendations based on the individual's specific circumstances, priorities and functional needs.

Why Use an Independent Occupational Therapist?

Flexible Access to Specialist Input

Independent therapy can provide additional flexibility when clients are unable to access NHS services quickly or require input outside of traditional pathways.

I can offer appointments tailored to the client's needs and circumstances, helping to provide timely assessment and intervention where appropriate.

A Focus on Function and Real-Life Outcomes

Occupational therapy is about more than assessing a person's physical abilities. It is about understanding how an injury or condition affects the things that matter to them in everyday life.

This could include:

  • Managing personal care

  • Returning to work

  • Managing household tasks

  • Using the hands effectively

  • Returning to hobbies and leisure activities

  • Improving independence

  • Managing fatigue or reduced physical function

  • Finding alternative ways of completing everyday tasks

By focusing on meaningful activities and real-life goals, rehabilitation can be practical, relevant and easier for clients to incorporate into their daily lives.

Specialist Upper Limb and Hand Therapy Experience

Where a client has an upper limb or hand injury, specialist assessment can help identify how their symptoms are affecting their function and what rehabilitation may be appropriate.

This may be particularly useful for clients experiencing difficulties with pain, stiffness, weakness, reduced range of movement, reduced grip strength or impaired dexterity.

I can provide assessment and rehabilitation aimed at improving functional use of the upper limb and helping clients work towards their individual goals.

Additional Support for Complex Cases

Clients with complex injuries or multiple conditions may benefit from input from an occupational therapist who can look at the whole person and how their different difficulties interact with everyday function.

I can provide an additional perspective alongside the existing multidisciplinary team and help identify practical barriers to independence and participation.

Clear Communication and Collaborative Working

Effective communication between professionals is an important part of successful rehabilitation.

I aim to work collaboratively with case managers and other professionals involved in a client's care, providing clear feedback regarding:

  • Assessment findings

  • Functional abilities

  • Rehabilitation progress

  • Barriers to progress

  • Recommendations for ongoing treatment

  • Equipment or adaptations that may be beneficial

  • Further therapy or onward referral where appropriate

This can help case managers maintain a clear understanding of the client's rehabilitation journey and make informed decisions about their ongoing care.

Supporting the Client's Rehabilitation Journey

My role is not simply to provide a therapy session and leave the client to continue independently. Where appropriate, I can help clients understand their difficulties, develop practical strategies and build confidence in managing everyday activities.

Rehabilitation may involve a combination of education, practical activity, therapeutic exercise, functional practice and strategies to improve independence.

The exact approach will depend on the individual's presentation, goals and response to treatment.

Working in Partnership With Case Managers

I understand that case managers need reliable professionals who can provide appropriate clinical input while also communicating effectively with the wider team.

My aim is to make the process as straightforward as possible by providing professional, evidence-informed and individualised occupational therapy input, with recommendations that are realistic and focused on the client's functional goals.

I can work with case managers on a one-off assessment basis, for a defined course of rehabilitation, or through ongoing review appointments, depending on the client's needs.
